But delivering great support isn’t just about what happens in person - it also entails documenting what occurred in the service, this is an essential part of your role.

That’s where shift notes come in. They help others understand how the client is progressing (or not), document what happened during the shift, support continuity of care, highlight any issues, and identify what’s working. Most importantly, they ensure that support is continuously adjusted to improve the client’s wellbeing and provide a clear record that supports both clients and you, the workers.
